Failed to maintain spill control equipment. Two yellow spill pallets located in the chemical storage <br />"The Cage" area have some spilled motor oil. The purpose of spill pallet is to catch any spills released <br />from containers and not to store hazardous substances. All facility communications/ alarm systems, fire <br />protection equipment, spill control equipment, decontamination equipment shall be tested and <br />maintained as necessary to assure its proper operation in time of emergency. Correct immediately by <br />removing the hazardous substance from spill pallets and handle the substance according to Hazardous <br />Waste regulations, California Code of Regulations, Title 22. It is recommended to place spill pads in the <br />pallets. Once spill pads are saturated, properly disposed the pads. <br />808. Failed to label universal waste to identify type of universal waste. Two cardboard boxes located in <br />Maintenance Shop lacked the identification of fluorescent tubes. Viewing the fluorescent tubes in the <br />box, it is difficult to differentiate used from new tubes. Per Maintenance Supervisor, they are all waste <br />fluorescent tubes. Universal waste must be properly labeled with the type of waste. Immediately label <br />container as "Universal Waste -fluorescent tubes". Universal waste can remain on site for up to one year <br />of start accumulation date. Ensure to retain disposal receipt on site for at least 3 years. <br />- Corrected on site. Safety Leader marked boxes. <br />Submit to SJC EHD within 30 days (05/04/12): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for each of the above violations <br />Notes: <br />- Quality Control lab uses only rubbing alcohol for sanitation of bottle caps for bacterial testing.
