Manifest number 008642791 JJK was found without a signed off copy from the destination <br /> facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination <br /> facility on site for three years and have them readily available for review. If the generator did not receive <br /> a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the <br /> generator's waste was submitted within 60 days of the date the waste was accepted by the initial <br /> transporter, the generator shall submit a legible copy of the missing manifest, with some indication that <br /> the generator has not received confirmation of delivery. <br /> This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to <br /> DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information <br /> submitted to DTSC by April 28, 2014. <br /> 135. The complete Return to Compliance paperwork for the March 24, 2011 inspection was not <br /> submitted. A deficiency letter was issued on October 15, 2013. The operator of the site or facility which <br /> receives an inspection report shall submit a written response to the department or the local officer or <br /> agency within 60 days of receipt of the inspection report, or with a shorter time as the department or <br /> local agency requires. This response shall include a statement documenting the corrective actions that <br /> have been or will be taken by the operator. Immediately submit all necessary follow up paperwork for <br /> the 2011 hazardous waste generator inspection to the EHD. <br /> 804 & 808. An open cardboard box containing used fluorescent light bulbs was observed unlabeled <br /> inside the maintenance shop. According to Mr. Eliason, tubes were placed in the box in October of <br /> 2013. A generator shall label or mark universal waste with the words 'Universal Waste' and the type of <br /> waste (batteries, mercury containing equipment, lamps/bulbs, electronic devices, CRTs, CRT glass, <br /> aerosol cans). The container was sealed and labeled appropriately at the time of inspection. Ensure that <br /> all universal wastes are labeled with the required information.
