Multiple 8-foot long used fluorescent bulbs (including broken bulbs) were observed in <br /> the rafters of the maintenance shop. One bulb shattered during the attempt to containerize these bulbs. <br /> The debris were swept and containerized inside a plastic bucket pending disposal. Universal waste <br /> lamps shall be contained in a closed container or package that is structurally sound, adequate to prevent <br /> breakage, and compatible with the contents of the lamp. A generator shall label or mark universal <br /> waste with the words 'Universal Waste' and the type of waste (batteries, mercury containing equipment, <br /> lamps/bulbs, electronic devices, CRTs, CRT glass, aerosol cans). Used fluorescent bulbs were <br /> containerized, sealed and labeled appropriately at the time of inspection. Ensure that all universal <br /> wastes are labeled with the required information. Since it cannot be determined how long these bulbs <br /> have been onsite, immediately dispose of them (and any other used fluorescent bulbs that were onsite <br /> for more than 1 year) per the Title 22 hazardous waste regulations, and submit a copy of the disposal <br /> record to the EHD by April 28, 2014. <br /> MINOR-Notice to Comply: 810, 812 <br /> 810. Based on site observations, the facility failed to provide initial or annual training to personnel who <br /> manage universal waste. Immediately begin training the onsite personnel who manages universal waste <br /> and submit a copy of training records to the EHD by April 28, 2014. <br /> 812. Copies of universal waste disposal records were not found on site. A generator of universal waste <br /> (batteries, mercury containing equipment, lamps/bulbs, electronic devices, CRTs, CRT glass, aerosol <br /> cans) shall keep a record of each shipment of universal waste on site for at least three years.
