Three waste oil containers and one waste coolant container were not labeled. Complete <br /> hazardous waste labels were located on the secondary containment. Label each container with the <br /> following six items. All hazardous waste containers shall marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all <br /> the required information. <br /> An emergency coordinator and modified contingency plan information is lacking. There must be <br /> at least one a each/ coordinator on site or on call to coordinate emergency response measures, and <br /> the following information mus d by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spI ment, and if present, fire alarm, and the <br /> phone number of the fire department, unless the facility has a rm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form ed that can be <br /> used for this purpose. Submit proof of correction to the EHD by October 5, 2013. <br /> C` <br /> Notes <br /> 1. Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, to the San Joaquin County <br /> Environmental Health Department, by October 5, 2013. <br /> 2. Go to cers.calepa.ca.gov on the internet, create an account and enter in the business information <br /> starting with the business activities section. This is a new mandatory state program. Call this office with <br /> any questions. <br /> 3. The facility operating permit fees are overdue.
