SUMMARY <br /> On May 29, 2002, the referenced facility was mailed a letter stating they needed to <br /> perform their annual monitoring certification line leak detector test by June 29, 2002. <br /> (Attachment 1) The required tests were not performed. <br /> On July 1, 2002,Dennis Catanyag, RENS,performed a routine underground storage tank <br /> (UST) and hazardous waste inspection. At the time of inspection the facility was given a <br /> report (Attachment 2) stating they must perform their annual monitoring certification, <br /> line leak detector and line tightness test (if applicable)by July 1, 2002. The UST <br /> inspection report informed the facility owner the following documents needed to be <br /> submitted: Current state form B, Certification of Financial Responsibility and Letter from <br /> the Chief Financial Officer, and Monitoring& Response Plan. None of the requested <br /> documents have been submitted. A hazardous waste inspection report was also left with <br /> the owner. (Attachment 3) The report told the facility to obtain an EPA identification <br /> number, obtain spill control equipment, and complete a hazardous waste emergency <br /> information form. No response has been received from the facility. <br /> On September 30, 2002, notification letters were mailed to the facility and owner stating <br /> secondary containment testing must be completed by January 1, 2003. (Attachment 4) <br /> On November 15, 2002 a certified letter was mailed to the facility stating it was their <br /> final notification to perform secondary containment testing by January 1, 2003. <br /> (Attachment 5) In addition numerous CUPA newsletters have been mailed to the facility <br /> stating the secondary containment-testing deadline. (Attachment 6) <br /> On December 26, 2002 Champion Precision Tank Testing notified the Environmental <br /> Health Department(EHD) secondary containment testing was scheduled for February 21, <br /> 1 <br />
