RESPONSE PLAN FOR MITIGATION OF UNAUTHORIZED <br /> RELF.A.SE FROM SINGLE-WALL TANK <br /> As required by 23 CCR, Div. 3, Chapter 16, Article 4, 2641(H) <br /> 1. Upon recognition of a suspected unauthorized release from the single- <br /> wall tank or product line (system) , store personnel will notify the <br /> Gasoline Compliance Manager at (602)530-5089 or pager no. (800)946-4646 <br /> pin no. 229-6801. <br /> 2. The Gasoline Compliance Manager will arrange for a qualified <br /> contractor to verify the suspected breach of the system and/or <br /> verify proper operation of the monitoring system. <br /> 3. Upon confirmation of loss of system integrity, the system will be <br /> taken out of service. Product will be removed from the tank if loss <br /> is from tank. <br /> 4. The Gasoline Compliance Manager will authorize all work necessary <br /> under this plan. Representatives of the repair contractor will conduct <br /> an inspection of the source of release to determine repairability. <br /> 5. All repairs to the failed system will be completed pursuant to the <br /> requirements of 23 CCR, Section 2661, and by or under the direct super- <br /> vision of the repair contractor. <br /> 6. Upon completion of repairs, the system will be precision tested in <br /> order to insure integrity. <br /> 7. Contaminated materials will be properly transported and disposed of <br /> under manifest. <br /> 8. Reactivate monitoring system prior to bringing the UST system back <br /> into service. <br /> Questions regarding this response plan should be directed to Myron Smith, <br /> Gasoline Compliance Manager, at (602) 530-5089. <br />
