The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 3. See pages 7 & 8 of the permit application for the required number of samples <br /> and analytical tests to be performed. Form EH 23 046 (Revised 7/22/10) must <br /> be utilized. <br /> 4. If the backfill material and soil are found to be contaminated it must be disposed <br /> of at an approved disposal location. Treatment of soil on site can only be <br /> performed under a hazardous waste treatment permit. <br /> 5. The excavation must be secured if it will not be backfilled immediately. <br /> 6. A copy of the "Site Health & Safety Plan" must be on the job site available for <br /> review. <br /> 7. A copy of the fire department permit must be submitted at the time of the <br /> removal. <br />
