8. The following information must be submitted to the EHD within thirty days after <br /> the UST removal date: <br /> i. Only analytical test results that come directly from a third party <br /> California certified laboratory with an affixed chain of custody will <br /> be accepted by the EHD. <br /> ii. The UST Tracking Record Sheet(s) and/or Tank Hazardous <br /> Waste Manifest(s). <br /> iii. Tank Closure Certification Form (DTSC Form 1249). <br /> iv. Hazardous Waste Manifest or disposal receipt for piping & other <br /> related appurtenances, waste liquid, residual fuel, waste oil, etc. <br /> 9. Contact Muniappa Naidu at(209) 468-3439 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. UST removal, UST pit and stockpile sampling. <br /> 10. Permit expires 180 days from the date of approval.
