The <br /> missing alarm includes: L18 alarm 3-21-2017 . During the monthly inspection, the designated operator shall review <br /> the alarm history for the previous month, check that each alarm was documented and responded to appropriately, <br /> and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> Submit proof of correction to the EHD. <br /> This is a Class II violation. <br /> 211 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Asia Buckley was performed on 5-9-2017 and her hire date was <br /> listed as 3-1-2017. The designated operator shall train facility employees for which he or she is responsible in the <br /> proper operation and maintenance of the UST system once every 12 months. The initial training shall be conducted <br /> within 30 days of the date of hire. <br /> The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3, Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site. <br /> This is a Class II violation. <br /> 301 HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> Stained oily pavement was observed at the satellite dispenser 11 at the time of inspection . The underground <br /> storage tank system shall be operated to prevent unauthorized releases. The nozzle was tightened and absorbent <br /> was applied to stained surface at the time of inspection, waste absorbent was placed into the onsite hazardous <br /> waste container pending disposal. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 307 CCR 2632(c) Monitoring system of the interstitial space of a double walled system not maintained. <br /> The racing fuel annular sensor failed to activate an audible and visual alarm when tested and, therefore, the tank is <br /> not monitored. The interstitial space shall be monitored with a monitoring system connected to an audible and <br /> visual alarm system.
