MEMO
DATE: September 18, 1993
T0: Lloyd Gums
731 Birchwood Drive
Lodi, CA 95240
FROM: PHS-EHD
P.O. Box 2 COPY
Stockton, Caa 95201
Eric Trevena
RE: 303 S. Cherokee Lane, Lodi
I Received your letter dated August 9 1993, regarding a closure letter for the property referenced above.
This memo is intended to clarify the options you have for obtaining a site status letter.
Below I have outlined three letters that may be sent from this office. If you decide to do further
investigation you may be required to obtain a permit and pay applicable fees through Public Health
Services-Environmental Health Division (PHS-EHD). Let me know which option you would like to pursue.
Letter A: •UST UNKNOWN'
This letter would be sent to the property owner if current standards, Laboratory detection limits and/or
your site specific characteristics make it impossible for PHS-EHD to evaluate your site for closure.
You would receive this letter if you choose to do no further investigation or even if you decide to remove
and sample the existing product lines.
Letter B: 'UST CLEAN'
This letter would be sent to the property owner if the soil results and field observations do not indicate
contamination.
You would receive this letter if you choose to remove and sample existing product lines and resample all
previous sample locations.
Letter C: "UST DIRTY"
This letter would be sent to the property owner it the soil results or field observations indicate soil
contamination.
You would receive this letter if any soil results or field observation indicate soil contamination.
Should you have any questions regarding this memo, please call me at (209) 468-0333.
