I <br />The IT Corporation medical surveillance program examination consists of. <br />• Medical and Occupational History Form (detailed questionnaire for new <br />employees, short questionnaire for periodic exams) <br />• Physical Examination <br />• Complete blood count with differential <br />• SMAC 20 or equivalent <br />• Urinalysis (dipstick and microscopic) <br />• Pulmonary Function Test (FEV/FVC) <br />• Audiogram <br />• Electrocardiogram for persons over 45 years of age, or if medically indicated <br />• Drug and alcohol screening <br />• Visual activity <br />• Follow-up exams. <br />All on-site subcontractor personnel who have the potential for exposure to hazardous <br />materials shall have successfully completed a medical examination similar to the preplacement <br />physical. A list shall be available of field employees and the date of their last physical <br />examination in accordance with the medical surveillance plan. <br />3.8.2 Medical Records <br />Medical and personnel exposure monitoring records will be maintained in accordance with the <br />requirements of 29 CFR 1910.20. Employee confidentiality shall be maintained. These <br />records shall be kept for 30 years. <br />3.9 Air Monitoring <br />Air monitoring shall be performed in the breathing zone around each excavation location. A <br />photoionization detector (PID) with a 10.2 eV probe will be used for air monitoring during <br />MZ107-07-92/EES/ENG/2054r 3-12 <br />
