<br /> 126 HSC 25404(e)(4) Failed to report program data electronically.
<br /> A submission to the California Electronic Reporting System (CERS) for the underground storage tank(UST) program is
<br /> not complete nor currently updated. A current response plan was not uploaded into CERS. The facility portion of the
<br /> CERS submittal was not accepted in CERS because it did not indicate"yes"to being a hazardous waste generator
<br /> under the business activities section. The ATG probe model number was listed as TLS 350 in the UST tank information
<br /> monitoring plan section. The correct model number for the ATG probe should be listed in CERS. Beginning January 1,
<br /> 2014, all businesses are required to submit all new(or any changes to existing) UST information, including: UST
<br /> Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator Written
<br /> Agreement(if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator
<br /> Compliance online to the CERS at http://cers.calepa.ca.gov. Be sure to include any other relevant activities and
<br /> required fields. This is a repeat violation, Class II.
<br /> 203 CCR 2712(b) Failed to maintain records of monitoring, repair, lining, and upgrades on site.
<br /> A February 4, 2014, probe out alarm did not have maintenance records documenting how the alarm was addressed.
<br /> The records were not available for review upon request. These records shall be kept on site for the life of the tank.
<br /> Immediately locate and maintain these records on site and submit copies to the EHD. This is a minor violation.
<br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed.
<br /> The designated operator failed to check and note the correct test dates for the last monitoring system certification,
<br /> secondary containment test, and spill containment structure testing on the October 2014, September 2014,August
<br /> 2014, July 2014, June 2014, May 2014,April 2014, and February 2014 designated operator monthly inspection reports.
<br /> The October 2014 and February 2014 designated operator report did not note the last monitoring system certification
<br /> test date of November 8, 2013. The designated operator report for September 2014,August 2014, July 2014, June
<br /> 2014, May 2014, and April 2014 did not note the correct date for the monitoring system certification. It was noted as
<br /> November 5, 2013. It should have been noted as November 8, 2013. The designated operator report for October 2014,
<br /> September 2014, August 2014, July 2014, June 2014, May 2014,April 2014, and February 2014 had the incorrect last
<br /> secondary containment test date and last spill containment structure test date. The noted test date for the secondary
<br /> containment test was November 9, 2011. The noted test date for the spill containment structure test was November 5,
<br /> 2013. The correct test date for the secondary containment test is November 7, 2011, and the correct test date for the
<br /> spill containment structure test is November 8, 2013. During the monthly inspection, the designated operator shall
<br /> check that all required testing and maintenance for the UST system have been completed and document the dates
<br /> they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the
<br /> required information on the reports. This is a Class II violation.
<br /> 212 HSC 25281.5(b)(3) Unburied emergency generator fuelI in not visually inspected at least monthly or log kept.
<br /> A log of monthly checks of the unburied single walled piping was not found on site. For an emergency generator tank
<br /> system, the unburied piping is exempt from regulation if visual inspections are made each time the system is operated,
<br /> but no less than monthly, and a log is maintained. Immediately begin inspecting the system each time the system is
<br /> operated, but no less than monthly, and maintain a log of the inspections, or submit a permit application to upgrade the
<br /> UST system to meet Title 23 UST regulations. This is a repeat violation, Class Il.
