AIRPORT WAY <br /> STOCKTON, CA 95205 <br /> I. CASE SUMMARY <br /> On November 13, 1987, Doug Wilson, Supervising REHS, authorized billing for the unregistered <br /> underground storage tank (UST) discovered at 442 N. Airport Way in Stockton. Registration fees <br /> for 1986 and 1987 were paid December 17, 1987. On May 16, 1988, Kasey Foley mailed the <br /> property owners (Don and Celia Nathe) information regarding UST Monitoring Alternative <br /> Requirements. The UST passed its first precision test on June 13, 1988. <br /> During the routine inspection on February 12, 1990, Dimetra Papadogiannis, EHS, found no <br /> weekly monitoring (known as tank gauging) of the UST was being performed, and the annual <br /> precision test for 1989 had not be done. The tank was precision tested and passed the following <br /> day on February 13, 1990. Monitoring was also initiated after the February 12, 1990 inspection. <br /> In 1992, San Joaquin County Public Health Services Environmental Health Division (EHD) UST <br /> billing included an UST information sheet outlining the December 22, 1998 UST upgrade deadline. <br /> These information sheets were sent to the owner or operator (whomever the billable party was <br /> according to the State "A" form) annually between 1992 and 1996. The billing for this facility was <br /> mailed directly to the facility. <br /> During the May 25, 1994 inspection, Letitia Briggs, Senior REHS, requested Financial <br /> Responsibility information from UST operator Ray Lopez. On July 14, 1994, Briggs reinspected <br /> the UST site for compliance and found Financial Responsibility information had not been <br /> completed by Mr. Lopez. Briggs sent copies of both Inspection Reports and a Financial <br /> Responsibility form to the property owner, Mr. Donald Nathe. On October 14, 1994, Briggs <br /> received the completed Financial Responsibility form from Mr. Lopez. <br /> In July of 1997, the EHD sent a package to UST owners or operators (whoever the billable party <br /> was according to the State "A" form) informing them of upcoming UST Upgrade Workshops that <br /> were to be held in the area by the State Water Resources Control Board (SWRCB). In that <br /> package, the EHD sent the survey provided by the SWRCB asking UST owners to let the EHD <br /> know of their plans to upgrade their tank(s). This package was mailed to the facility. EHD didn't <br /> receive a completed survey from this facility. <br /> On November 4, 1997, the EHD sent a letter to UST owners or operators (the billable party <br /> according to the State "A" form) notifying them of an UST Upgrade Workshop to be held in <br /> Stockton sponsored by the EHD and the SWRCB discussing the upcoming December 1998 UST <br /> upgrade deadline. <br /> A Division of San Joaquin 6unty Health Care Services <br />
