I have placed Costco's return to compliance response to each issue in parenthesis: #3: Current CUPA Facility & Tank Forms (attached forms). #4:Current Fin. Resp. Forms (attached forms). #5:Monitoring & Response Plans not current (attached forms). UDC 5/6; 7/8 & 9/10 sensors were not operating as per the monitoring plan. 5/6 & 7/8 were reprogrammed prior to the completion of the inspection. 7/8 was replaced by Elite IV on 8/18/09. (Elite IV work order #22398 attached). #23: Designated Operator Notification form was not sent to Agency prior to DO inspections (in progress). #32: CPM performed cold starts on 12/3/08 and 1/21/09. A permit is required for this work. Immediately apply for a double permit (Permit application is in progress). #38: UDC sensor 7/8 (1-23) would not come out of alarm (Elite IV work order #22398 is attached).
