I have placed <br />Costco's return to compliance response to each issue in parenthesis: <br />#3:Current CUPA Facility & Tank Forms (attached forms). <br />#4:Current Fin. Resp. Forms (attached forms). <br />#5:Monitoring & Response Plans not current (attached forms). <br />UDC 5/6; 7/8 & 9/10 sensors were not operating as per the monitoring plan. 5/6 & 7/8 were <br />reprogrammed prior to the completion of the inspection. 7/8 was replaced by Elite IV on 8/18/09. (Elite IV <br />work order #22398 attached). <br />#23: Designated Operator Notification form was not sent to Agency prior to DO inspections (in progress). <br />#32: CPM performed cold starts on 12/3/08 and 1/21/09. A permit is required for this work.
