Charter Way, Stockton CA <br /> The permit (SR0052391)to repair the failures for the underground storage tank system(UST) at <br /> the above referenced site has been approved with the following conditions. <br /> Conditions of approval: <br /> 1. Contact the local Fire Department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD) for approval prior to commencing work. All new addenda supercede <br /> the previous ones. <br /> 3. All components must be certified by an independent testing organization. (UL, ULC, <br /> ANSI,API, ASTM, etc. See California Code of Regulations (CCR),Title 23 Chapter 16 <br /> Table B for complete list). The component must bear the appropriate mark as required by <br /> the independent testing organization. <br /> 4. If concrete is broken near the UST,a third party tank integrity test and/or line integrity <br /> test(or equivalent)must be performed, once concrete is poured in accordance with the <br /> manufacturer's guidelines.Results must be received by EHD within seven days of <br /> returning the facility to operating condition. <br /> 5. Continuous monitoring must be maintained for the UST during the investigation of the <br /> water intrusion/infiltration/release into the annular space of the 87 and 91 tanks. <br /> 6. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid,current California UST certificates issued by the International Code <br /> Council (ICC),that are applicable to the work being performed, and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested, repaired, or serviced. <br />
