The piping for the 87 B tank in <br /> dispensers 1/2 and 7/8 is set up to cross from one side of the UDCs to the other, which places 87 fuel piping over all 3 <br /> Bravo float and chain compartments in the UDC. This set up prevents the 87 B shear valve from being tripped by leaks <br /> in the 87 B piping over the 87 A(blanked)and 91 octane float and chain compartments at the earliest opportunity. <br /> Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. The barriers <br /> between compartments in the Bravo boxes in UDCs 1/2 and 7/8 were removed at the time of inspection. Ensure that all <br /> monitoring equipment is maintained to detect a leak at the earliest opportunity. This is a Class II violation. <br /> 309 CCR 2636(%1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The Bravo box float and chain in the dispenser 3/4 (91), 5/6 (91), and 7/8 (87) UDC sumps failed to stop the flow of <br /> product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual <br /> alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the chain(s) <br /> and verified functionality. This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 13, 2014. <br /> Consent to perform the inspection, take photos and collect samples was given by Ravinder Singh, owner. This <br /> facility's designated operator is Alex Jabbari (ICC expiration date: 10/3/16) . The service technician was Stephen <br /> Willems (ICC expiration date: 6/2/16), who had the following manufacturer's certifications: Veeder-Root (exp: <br /> 7/8/16), VMI Tester (exp: 10/1/16). <br /> NOTE: According to maintenance records on site, the 87 turbine was replaced on November 29, 2013. Leak <br /> detector test results were not submitted to the EHD after this replacement. The EHD does not require a <br /> retrofittrepair permit for turbine repairs or replacements, however, the leak detector must be certified by a properly <br /> licensed service technician, and passing test results submitted to the EHD within 30 days of testing.
