MAY-21-96 16:41 FROM=WESTON NCA ID Sinonsr r„ <br /> • Materiallwaste spilled and its quantity. <br /> • Area affected. <br /> • Time that spill was first observed. <br /> • Response actions taken. <br /> If there is a potential for contamination as a result of the release, this must also be <br /> reported. <br /> 3.0 FIRES AND EXPLOSIONS <br /> 3.1 Discovering the Fire/Explosion <br /> In the event that a fire or explosion occurs, the employee discovering the fire or explosion <br /> should immediately activate the store fire alarm (shown on Figure 1) and then call 911 on <br /> the nearest telephone (also shown on Figure 1) and provide the following information to <br /> the emergency attendant, speaking slowly and carefully: <br /> • Name and address of Pep Boys Store. <br /> • Type of Emergency that has occurred. <br /> • His/her name. <br /> • Any injuries that may have occurred. <br /> • Name and type of hazardous materials/wastes involved,to the extant known. <br /> • Any other information requested. by the emergency attendant. <br /> 3.2 Store Evacuation <br /> After hearing the store fire alarm, all employees are to calmly and quickly escort all <br /> customers to the nearest emergency exit using the emergency routes shown on Figure 1, <br /> and to the evacuation area (Figure 1), making sure that all customers exit the store. If <br /> exits are blocked by the fire or explosion, do not use them and instead use an alternate <br /> route and evacuation area as shown on Figure 1. Try to avoid any smoke or fumes, and <br /> stay up wind of the fire if possible. Customers are free to leave the site, but employees <br /> must stay in the evacuation area until they are told to return to work or go home by the <br /> Emergency Coordinator or are threatened by the fire or explosion. The Emergency <br /> Coordinator is responsible for determining if all customers and employees have exited the <br /> store, and shall report any missing employees to the emergency response crews. <br /> 3.3 Emergency Response and Equipment <br /> If the fire is small and fighting it does not pose a threat to the employee, the discovering <br /> employees may fight the fire, but only after the evacuation slam] has been given over the <br /> intercom, the Emergency Coordinator has been alerted, and the employee is sure that the <br /> evacuation has begun_ Figure 1 shows the locations of fire fighting equipment at the <br /> PEP_BOYS.DOC 6 5120196 <br />
