Financial responsibility documents have not been submitted to the EHD. The most recent copy on <br /> file is dated 5-1-11. Current financial responsibility documents are required to be submitted annually. <br /> During the inspection the employee provided a copy of the current financial responsibility documents <br /> dated 2-10-12. Continue to ensure that financial responsibility documents are required to be submitted <br /> annually. Corrected on site. <br /> 201. A facility alarm log and maintenance record for the 3-17-12 L4 diesel stp fuel alarm were not found <br /> on site. These records shall be kept on site for at least 3 years. Immediately locate and maintain the <br /> facility alarm log and maintenance record for the 3-17-12 L4 diesel stp fuel alarm on site and submit <br /> copies to the EHD by 6-15-12. Continue to ensure that alarms from the monitoring panel are <br /> documented by the facility and that maintenance records for those alarms are kept on site for three <br /> years. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> April 2012 designated operator monthly inspection report and failed to check that they were responded <br /> to appropriately. The missing alarms include: the 3-17-12 L4 diesel stp fuel alarm. During the monthly <br /> inspection, the designated operator shall review the alarm history for the previous month, check that <br /> each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> 303. The diesel annular sensor was found not located at the bottom of the tank to detect a leak at the <br /> earliest opportunity. Technician had to lower the Veeder Root-420 bell sensor about one inch so that it <br /> could reach the bottom of the tank annular. Monitoring equipment shall be maintained to be able to <br /> detect a leak at the earliest possible opportunity. Continue to ensure that sensors are properly located to <br /> detect a leak at the earliest opportunity. Corrected on site. <br /> 303 & 305. The 87 turbine sump sensor is not working properly to ensure continuous monitoring of the <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector: Received By, Title. <br /> EHD 23-02 Rev 49/01111 Page 3 UST INSPLCTION REPORT <br />
