Violett <br />12/27/93 <br />Following is the "Written Routine Monitoring Procedure" for Polar <br />Water Co. as required by Ca. UST Regs, Sec. 2632(e)(1). <br />We maintain two 1,000 gallon tanks, one for diesel, and one for <br />unleaded gasoline. Monitoring is done on a daily basis, both <br />visually, and by Petrometer annular space monitor, which operates <br />24 hours a day. Visual monitoring is done at the pumps, and also <br />by checking the monitor alarm which is situated adjacent to the <br />pump shut off switches. Said monitoring is done at the end of the <br />day when shutting off the pumps, and is logged at that time. A <br />sample copy of the log sheet is enclosed. <br />Lockup of our facility and reporting of monitoring, is effected <br />by the following persons: <br />1). Robert Haase Manager <br />2). Robert Miloslavich Officer of Corp. <br />3). Gary Ghan Rt. Supervisor/Safety Coordinator <br />4). Greg Levitt Asst. to Rt. Supervisor <br />5). Mike Koenig Plant Manager <br />Training for use of fuel pumps and monitoring equipment is <br />conducted by plant manager and head mechanic Mike Koenig, and rt. <br />supervisor/safety coordinator Gary Ghan. Every new hire is given <br />personal instruction regarding safe use of both diesel and <br />unleaded gasoline fuel pumps. Key lock system, hose and nozzle <br />use, shut-off switches both at pump and remote are demonstrated. <br />We stress the necessity to stay with the vehicle while fueling, <br />and outline steps to take should a spill occur. Refresher <br />instruction is given to all employees with access to fuel <br />annually as minimum. When it is necessary to train someone for <br />the monitoring system, Robert Haase, Gary Ghan or Greg Levitt do <br />SO. <br />When supplier delivers fuel(s), someone is present at all times <br />and product is metered. <br />Response Plan: <br />In the event of a leak in one of our primary tanks, the <br />Petrometer annular space monitor alarm would alert us to effect <br />the following response: Without delay, one of the authorized <br />employees noted above would call for immediate service to: Jim <br />Thorpe oil, the contractor who installed our UGST's. <br />P.O.
