The monitoring plan is not current and/or not approved by the EHD. The monitoring plan uploaded on March 20, 2015 listed Red Jacket line leak detectors, however, VMI line leak detectors were observed onsite for both tanks. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a Class II violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted, the submitted plan doesn't identify the sensor/monitoring locations. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class IL 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. Secondary containment testing was performed on May 27, 2015 and a test report was submitted on March 6, 2017, (a copy of the secondary containment test report was received at the time of the inspection),10 months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This was corrected on site. This is a repeat violation, Class Il. 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. Annual monitoring system certification, leak detector testing, and spill container testing was performed on March 17, 2016 and a test report was received on June 3, 2016, 3 months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class II violation. 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on March 17, 2016 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by April 17, 2016. To date only a partial Return to Compliance inspection report was received and several violations are outstanding. This outstanding information has not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a repeat violation, Class ll.
