Monitoring records (monitoring <br /> system certification for March 17, 2016 were not found on the site. These records shall be maintained on site for at <br /> least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring <br /> equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis <br /> performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water <br /> elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and <br /> maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> This is a Class II violation. <br /> 205 CCR 2712(i) Failure to have an approved UST response plan available onsite. <br /> An approved copy of the response plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> This is a Class II violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to check and note the correct test dates for the secondary containment testing from <br /> March 2016 to February 2017 designated operator monthly inspection reports. The above test was listed as May <br /> 28, 2015 instead of May 27, 2015. During the monthly inspection, the designated operator shall check that all <br /> required testing and maintenance for the UST system have been completed and document the dates they were <br /> done. Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a Class II violation. <br /> 310 CCR 2636(f)(5) Piping is not fail-safe outside UDCs or fails to stop flow of product at dispenser inside UDCs. <br /> The 91 STP sump sensor failed to shut down the turbine when tested and did not meet the requirements for annual <br /> line tightness testing exemption. The service technician applied force to the 91 STP sump relay box causing the <br /> shut down, the 91 STP sump sensor was tested with passing results, which satisfies the annual line tightness <br /> testing requirement. If the issue persist repair under permanent inspection from the EHD.
