EPA IDENTIFICATION NUMBER FACT SHEET <br />Permanent EPA Identification numbers are required by Federal and State laws <br />for transporters, Treatment Storage and Disposal Facilities (TSD's) and <br />generators who produce a hazardous waste. The generator and disposal <br />facility numbers are site specific. This means that if your business moves <br />to a new location you must notify EPA of the closure of the old location <br />and obtain a number for the new location of your business. The <br />application form is EPA form 8700-12, "Notification of Hazardous Waste <br />Activity". A responsible representative of the company must sign this form <br />in order for a EPA ID number to be issued. The turn -around time for <br />issuance of permanent numbers is four to six weeks. Under justifiable <br />circumstances we may acknowledge your number sooner. In order to do so you <br />must attach a cover letter to the application explaining your situation. <br />Issuance of provisional (1 -time) numbers takes ten working days. These EPA <br />ID numbers are not permanent and are flagged as such in our database. <br />These numbers are given out only in one-time generator situations, i.e. <br />closure of a business, underground tank removal or asbestos removal. <br />FOR ONE-TIME REMOVAL OF ASBESTOS (RESIDENTIAL AND COMMERCIAL) OR <br />UNDERGROUND TANK REMOVAL, THE GENERATOR MAY CALL THE STATE FOR AN EPA <br />NUMBER OVER THE PHONE - (916) 324-1781. <br />EMERGENCY EPA ID NUMBERS ARE GIVEN OVER THE PHONE BY EPA ONLY. NOTE: <br />THESE NUMBERS ARE FOR SPILLS AND CRISIS SITUATIONS WHICH THREATEN <br />HUMAN HEALTH OR THE ENVIRONMENT. EPA MUST SPEAK TO A REPRESENTATIVE <br />OF A RESPONDING AGENCY OR FIRE DEPARTMENT AND TO THE GENERATOR OF THE <br />WASTE. THESE NUMBERS ARE TO BE USED ONLY ONCE. The first <br />two letters represent the state (e.g., CA, AZ, NV) the third letter is a <br />"D" or "T".
