The following items still need to be
addressed:
1. You need to contact the San Joaquin County Fire Prevention
Bureau at (209) 468-2229 to obtain a fire permit.
2. The closure application indicates Wright Environmental as
the sampling firm. A third party, independent of the tank
owner and the contractor removing the tank, is required to
obtain and take the soil/water samples to the laboratory.
Please indicate who this firm will be.
3. You still need to obtain an EPA I.D. #, by phoning the State
of California at (916) 324-1781. Submit the number to this
office after you obtain it.
4. Section two of page ten needs to be accurately completed.
5. Underground storage tank back fees and penalties of $2,112
need to be submitted to this office.
If you have any questions regarding this matter please contact
Eric Trevep at (209) 468-0333.
A Division of San Joaquin County Health Care Services
