• <br />Haight Road Tank Removal <br />NOA Project Number: E07174A <br />January 2008 <br />6.4 Heavy Equipment Safety <br />• 12 <br />No heavy equipment use is anticipated. The following procedures will be implemented <br />in the event heavy equipment is required: <br />Any equipment defects that affect safety will be corrected by the tank removal <br />contractor or its subcontractors in a timely manner so a personnel hazard is not <br />created. When defects make continued operation hazardous to personnel, the <br />defective equipment will be taken out of service and placed in the designated area for <br />repair. Once tagged out, continued use of equipment is prohibited until the defects <br />are repaired. Defects on self-propelled mobile equipment affecting safety, which are <br />not corrected immediately, will be reported to the Field Team Leader. The Field <br />Team Leader will keep a log that will include the date the defect was reported, the <br />equipment's identification, a description of the defect, and the date of repair. <br />Equipment repairs or maintenance will be performed only after the power is off, and <br />the equipment is blocked against hazardous motion. Equipment motion or activation <br />is permitted to the extent that adjustments or testing cannot be performed without <br />doing so, provided that people are effectively protected from hazardous motion. <br />operators of self-propelled mobile equipment will maintain control of the equipment <br />while it is moving. Operating speeds will be consistent with conditions of roadways, <br />grades, clearances, visibility, traffic, and the type of equipment used. Equipment will <br />be operated at speeds that permit stopping in no more than half the visibility <br />distance. <br />People will not be transported: <br />• in or on dippers, clamshells or buckets <br />• in beds of mobile equipment <br />• on top of loads in mobile equipment <br />• outside cabs, equipment operator's stations, and beds of mobile equipment <br />• to or from work areas in overcrowded equipment (i.e., the vehicle will not <br />carry more people than the number of seats on that vehicle) <br />All self-propelled mobile equipment will be equipped with a service brake system <br />capable of stopping and holding the equipment with its typical load on the maximum <br />grade it travels (this does not apply to equipment that was not originally equipped <br />with brakes). If equipped, the parking brake on self-propelled mobile equipment will <br />be capable of holding the equipment under typical load condition on the maximum <br />travel grade. <br />All braking systems installed on self-propelled mobile equipment will be maintained in <br />a functional condition. <br />Front-end loaders and bulldozers should have protection from falling objects. <br />
