0 <br />Haight Road Tank Removal <br />NOA Project Number: E07174A <br />January 2008 <br />Seat belts meeting the requirements of SAE J386, <br />Road Work Machines, 1985, will be provided and <br />will be maintained in functional condition and <br />proper performance. <br />13 <br />Operator Restraint Systems for Off - <br />worn in haulage trucks. Seat belts <br />replaced when necessary to assure <br />Mobile equipment will not be left unattended unless the controls are placed in the <br />park position, the parking brake, if provided, is set, and the ignition turned off. <br />People will not work on top of, under, or from mobile equipment in a raised position <br />until the equipment has been blocked or secured to prevent it from rolling or falling <br />accidentally. <br />Care will be taken to locate all overhead power lines before excavation begins. Under <br />no circumstances should any part of the mobile equipment be positioned within the <br />minimum clearance from exposed and energized electrical wires. The equipment <br />operator will ensure there is sufficient overhead clearance (i.e., no part of the <br />equipment will hit or touch any overhead obstruction when raised nor will it hit or <br />touch any object while being raised) before raising any part of the equipment through <br />careful preplanning. <br />6.5 Trenching and Excavation <br />No trenching or excavation activities are anticipated, except for hand augering for the <br />soil samples. Should any such work be desired, all operations involving excavation or <br />trenching will be performed in accordance with Cal/OSHA Regulations for excavations <br />(T8 CCR 1539 -1541.1). An excavation is considered to be any man-made cut, cavity, <br />trench, or depression in an earth surface, formed by earth removal. <br />No personnel will be allowed to enter an unshored or unsloped excavation that is <br />deeper than 5 feet. Personnel must obtain approval from the Project Health and <br />Safety Officer before entering any excavation. All excavation activities must be <br />performed at a minimum under the direction of a competent person. <br />Underground Utilities <br />The estimated location of utility installations, such as sewer, fuel, gas, phone cable, <br />communication cable, TV cable, electrical, potable/sanitary water, or any other <br />underground installations that reasonably may be expected to be encountered during <br />excavation work, shall be determined prior to opening an excavation. When <br />excavation operations approach the estimated location of underground installations, <br />the exact location of the installations shall be determined by safe and acceptable <br />means. <br />
