All accidents involving a death, multiple <br />personnel injuries, lost time injury, or substantial property damage will be <br />investigated. <br />The procedure may be directed by the attorneys to provide the most reliable evidence <br />or description legally permissible. All investigations pursuant to the directions of legal <br />counsel will be protected by all applicable privileges. The attorney will provide more <br />detail on this topic during the investigation. <br />11.0 CONFINED SPACES <br />No confined space entry is anticipated at the site. A confined space protocol will be <br />developed for Agency review and approval should conditions at the site change. <br />12.0 EXPOSURE MONITORING PLAN <br />Procedures for monitoring exposures to site workers and the community are discussed <br />in the following sections. <br />12.1 Air Monitoring <br />Because the project will not generate significant quantities of fugitive dust, air <br />monitoring is not proposed. <br />12.1.1 Site Dust Control and Monitoring <br />Because the project will not generate significant quantities of fugitive dust, dust <br />control procedures and monitoring are not proposed. <br />12.2 Noise <br />Use of hearing protection devices will be required whenever equipment is operated. <br />Hearing protection devices will be made available in the event that equipment use is <br />anticipated. <br />Noise from on-site activities is not considered a hazard to the surrounding community <br />because no excess noise will be generated from on-site activities. <br />12.3 Physical <br />Personnel will be visually monitored for signs of heat- or stress-related illnesses or for <br />any injury from site activities. <br />
