P 0 16 <br />Haight Road Tank Removal <br />NOA Project Number: E07174A <br />January 2008 <br />• Medicine and alcohol can exacerbate the effects from exposure to toxic <br />operations where the potential for absorption, inhalation, or ingestion of toxic <br />substances exists, unless specifically approved by a qualified physician. <br />Consumption of alcoholic beverages is prohibited. <br />• Adequate side and overhead clearance must be maintained to ensure the <br />equipment booms do not touch or pass close to any overhead power lines or <br />other overhead obstacles or obstructions. <br />• "Dig -Alert" representatives shall be asked to identify all underground utilities. <br />Utility lines should be marked using characteristic spray paint or labeled <br />stakes. <br />10.2 General Rules of Conduct <br />The following general rules of conduct are required for anyone working on this <br />project: <br />• Liquor, firearms, narcotics, tape recorders, and other contraband items are not <br />permitted on the premises. <br />• Any violation of local, state, or federal laws, or conduct outside the generally <br />accepted moral standards of the community is prohibited. <br />• Willfully damaging or destroying property, or removing records is forbidden. <br />• Misappropriation or unauthorized alteration of any record is forbidden. <br />• Gambling in any form, selling tickets or articles, taking orders, soliciting <br />subscriptions, taking up collections, etc., is forbidden. <br />• Compliance with posted signs and notices is required. <br />• Boisterousness and noisy or offensive work habits, abusive language, or any <br />oral, written, symbolic, or other communication that tends to disrupt work or <br />morale of others is forbidden. <br />• Fighting or threatening bodily harm to another is forbidden. <br />• Defacing any property is forbidden. <br />• Wearing shorts of any type and/or offensive logos, pictures, or phrases on <br />clothing is forbidden. Shirts, shoes, and pants, slacks, or coverall -type <br />garments will be worn at all times. <br />People operating motor vehicles will obey all traffic regulations. <br />10.3 Injury/Illness Reporting <br />OSHA Records Required <br />NOA will maintain copies of required accident investigations, copies of the OSHA 301 <br />and 300 Logs, hazard identification and abatement, and certification of employee <br />safety training. A written report will be maintained on each recordable accident, <br />injury or on-the-job illness. See Attachment A for Accident Report Form. A record of <br />each such injury or illness is recorded on OSHA Log and Summary of Occupational <br />injuries Form 300 according to its instructions. Supplemental records of each injury <br />are maintained on OSHA Form 301, or state -specific Employers Report of Injury or <br />Illness Form. Every year, a summary of all reported injuries or illnesses is posted no <br />later than February 1, for three months, until May 1, on OSHA Form 300A. These <br />
