Haight Road Tank Removal•
NOA Project Number: E07174A
January 2008
21
Contractor has evaluated the situation and provided the appropriate
instructions to the field team.
If an accident occurs, the Environmental Enginner is to complete an accident
report form for submittal to the appropriate company official.
If a member of the field crew suffers a personal injury, the Tank Removal
Contractor will call 911 (serious injury) to alert appropriate emergency
response agencies or administer onsite first aid (minor injury) as the situation
dictates. An Accident Report Form will be completed for any such incident.
If a member of the field crew suffers a chemical exposure, the affected areas
should be flushed immediately with copious amounts of clean water. If the
situation dictates, the Site Health and Safety officer should alert appropriate
emergency response agencies, or personally ensure that the exposed individual
is transported to the nearest medical treatment facility for prompt treatment.
An Accident Report Form will be completed for any such incident.
16.0 DIRECTIONS TO HOSPITAL
The closest hospital is:
Lodi Memorial Hospital
975 S Fairmount Avenue
Lodi, CA
209-334-3411
Directions
• Start out going West on E Haight Rd toward N Curry Ave. (<0.1 mites)
Turn LEFT onto N Curry Ave. (0.2 miles)
• N Curry Ave becomes E Armstrong Road (right).
