Haight Road Tank Removal• <br />NOA Project Number: E07174A <br />January 2008 <br />21 <br />Contractor has evaluated the situation and provided the appropriate <br />instructions to the field team. <br />If an accident occurs, the Environmental Enginner is to complete an accident <br />report form for submittal to the appropriate company official. <br />If a member of the field crew suffers a personal injury, the Tank Removal <br />Contractor will call 911 (serious injury) to alert appropriate emergency <br />response agencies or administer onsite first aid (minor injury) as the situation <br />dictates. An Accident Report Form will be completed for any such incident. <br />If a member of the field crew suffers a chemical exposure, the affected areas <br />should be flushed immediately with copious amounts of clean water. If the <br />situation dictates, the Site Health and Safety officer should alert appropriate <br />emergency response agencies, or personally ensure that the exposed individual <br />is transported to the nearest medical treatment facility for prompt treatment. <br />An Accident Report Form will be completed for any such incident. <br />16.0 DIRECTIONS TO HOSPITAL <br />The closest hospital is: <br />Lodi Memorial Hospital <br />975 S Fairmount Avenue <br />Lodi, CA <br />209-334-3411 <br />Directions <br />• Start out going West on E Haight Rd toward N Curry Ave. (<0.1 mites) <br />Turn LEFT onto N Curry Ave. (0.2 miles) <br />• N Curry Ave becomes E Armstrong Road (right). (2.2 miles) <br />• Turn Right onto N West Ln / CR -J3 N. Continue to follow CR -J3 N. (2.0 miles) <br />• Turn Left onto W Kettleman Ln / CA -12. (0.3 miles) <br />Turn Right onto S Fairmont Ave. (0.4 miles) <br />• End at 975 S Fairmount Ave. <br />The distance is about 5 miles and the time is about 10 minutes. <br />17.0 SPILL CONTAINMENT <br />The need for spill containment is not anticipated to be an issue for this project <br />because of the nature of the work. Should any hazardous material be spilled, it should <br />be contained as well as possible according to methods which do not place site <br />personnel at risk. <br />18.0 AUTHORIZED CHANGES TO THE HEALTH Et SAFETY PLAN <br />Any changes to this Health and Safety Plan must be approved in writing by the <br />Environmental Engineer. The documentation will become part of the project file. <br />
