RESPONSE TO GENERAL INSTRUCTIONS <br /> Item 1: The KJAX tank is a part of our national program which requires that standard <br /> subcontract procurement procedures are followed. We are in the process of selecting <br /> contractors and are sending pre-qualification packages to various tank removal contractors <br /> in order to obtain competitive bids. We have requested that the pre-qualification packages <br /> be returned to WCFS by May 3, 1993. We anticipate that a contractor will be selected by <br /> June 1, 1993. Our application will be updated at that time and the sections which require <br /> the specification of contractors for tank removal, waste hauling, disposal and other activities <br /> will then be completed. <br /> The contractor will conduct the required tank removal activities in accordance with <br /> California Code of Regulations Title 23, Chapter 16, Article 7, and applicable federal and <br /> local regulations. <br /> Item 2: A detailed site map is enclosed. The exact location of underground utilities is <br /> not known. USA Dig will be contacted prior to excavation activities. <br /> Item 3: See Item 1. <br /> Item 4: We are currently working with FEMA to obtain the required signatures. <br /> Item 5: We are currently working with the FEMA project manager to procure funding for <br /> the appropriate fees as calculated on page 11 of the application. We anticipate that payment <br /> for these fees will be provided with our updated application following the selection of a <br /> contractor. <br /> Item 6: Page 10 has been completed as directed. <br /> Item 7: WCFS has prepared a draft Health and Safety plan and will submit a final site <br /> specific plan to the SJCPHS with our updated application. <br /> Item 8: A backfill excavation certificate will be obtained following the selection of a tank <br /> removal contractor. <br /> Item 9: A grading permit is not necessary. <br /> Item 10: We understand that additional fees may be required for review of our updated <br /> application. We are currently working with the FEMA project manager to obtain a <br /> signature acknowledging these possible additional fees. <br /> Item 11: We will submit verification of the fire permit following selection of a contractor. <br /> Item 12: We will provide SJCPHS with 48 hours notice prior to tank removal activities. <br /> KJAX.CIS/06-21-93 SAC <br />
