The UST Owner/Operator: Written <br /> Agreement, if applicable, can be stored at the facility or uploaded into CERS. <br /> Notes: The service technician was not able to test vacuum Communication in the vent sump nor biodiesel line <br /> due to the lack of test valves.This test is required once every 12 months. Contact a properly licensed, trained, <br /> and certified contractor to add the test valves and make repairs under permit , if necessary, and inspection of <br /> the EHD. Schedule this test before the 12 month time frame expires to avoid late testing violation, provide 48 <br /> hours notification to the EHD.
