�M WESTERN METER SERVICE, INC. oMI UCT 9 1990 HEALTH and SAFETY PLAN ""`' ' °c N 1l HE TIME OIL/JACKPOT 'MIPSE?V' , The site location for the proposed job of removing six (6) tanks consisting of three (3) 1,000 gallon gasoline tanks, one (1) 6,000 gallon premium gasoline tank, one (1) 10,000 gallon unleaded gasoline and one (1) 12,000 gallon regular gasoline tank is as follows: TIME OIL/JACKPOT 13475 JACKTONE ROAD LODI, CA 95240 The project manager, and the health and safety coordinator is Wesley W. DuBose of Western Meter Service, Inc., 2735 Teepee Drive, Suite E. Stockton, Ca 95205, (209) 948-6124. The site safety officers at the job site will be Wes- ley W. DuBose and/or Dan Ensley of Western Meter Service, Inc. The alternate site safety officer will be John Swets of Western Meter Service. Inc. Western Meter Service, Inc. will have on the job site one (1) fire extin- guisher and one (1) first-aid kit within 50' of the tank location. See at- tached map. No smoking will be permitted within 50' of the tank or areas where flammable vapors are present. Western Meter Service, Inc. shall notify the local Fire Department 48 hours before excavation shall begin. USA will be notified before excavation of job. Tanks must be empty of product before start of the job. Western Meter Serv- ice, Inc. will triple rinse tanks with soap and water. Rinse will be hauled away by Refineries Service, Inc., Patterson, Ca 95363, (209) 892-6742, a waste hazardous hauler. Western Meter Service, Inc. will mail a copy of the rinse manifest to San Joaquin E.P.A. Tanks will be dry iced, 15 pounds of dry ice per 1,000 gallons before excavating. Western Meter Service, Inc. shall monitor tanks with a Combustible Gas Indicator (CGI) and seal all openings before tanks will be transported. At no time will Western Meter Service, Inc. cut or weld on the tanks. Tanks will be secured on truck after extraction for transportation. Tanks and plumbing will be hauled to by Redfern Trucking, 3736 S. Highway 99, Stockton, Ca 95205, (209) 948-0080 to Triangle Inc. of Sacramento, 3525 52nd Avenue, Sacramento, Ca 95823, (916) 421-1990 to be destroyed. All of the above procedures will be done according to code. Western Meter Service, Inc. will mail a copy of the tank manifest to San Joaquin E.P.A.
