�M WESTERN METER SERVICE, INC. <br />oMI <br />UCT 9 1990 <br />HEALTH and SAFETY PLAN ""`' ' °c N 1l HE <br />TIME OIL/JACKPOT 'MIPSE?V' , <br />The site location for the proposed job of removing six (6) tanks consisting of <br />three (3) 1,000 gallon gasoline tanks, one (1) 6,000 gallon premium gasoline <br />tank, one (1) 10,000 gallon unleaded gasoline and one (1) 12,000 gallon <br />regular gasoline tank is as follows: <br />TIME OIL/JACKPOT <br />13475 JACKTONE ROAD <br />LODI, CA 95240 <br />The project manager, and the health and safety coordinator is Wesley W. DuBose <br />of Western Meter Service, Inc., 2735 Teepee Drive, Suite E. Stockton, Ca <br />95205, (209) 948-6124. The site safety officers at the job site will be Wes- <br />ley W. DuBose and/or Dan Ensley of Western Meter Service, Inc. The alternate <br />site safety officer will be John Swets of Western Meter Service. Inc. <br />Western Meter Service, Inc. will have on the job site one (1) fire extin- <br />guisher and one (1) first-aid kit within 50' of the tank location. See at- <br />tached map. No smoking will be permitted within 50' of the tank or areas <br />where flammable vapors are present. <br />Western Meter Service, Inc. shall notify the local Fire Department 48 hours <br />before excavation shall begin. USA will be notified before excavation of job. <br />Tanks must be empty of product before start of the job. Western Meter Serv- <br />ice, Inc. will triple rinse tanks with soap and water. Rinse will be hauled <br />away by Refineries Service, Inc., Patterson, Ca 95363, (209) 892-6742, a waste <br />hazardous hauler. Western Meter Service, Inc. will mail a copy of the rinse <br />manifest to San Joaquin E.P.A. Tanks will be dry iced, 15 pounds of dry ice <br />per 1,000 gallons before excavating. Western Meter Service, Inc. shall <br />monitor tanks with a Combustible Gas Indicator (CGI) and seal all openings <br />before tanks will be transported. At no time will Western Meter Service, Inc. <br />cut or weld on the tanks. Tanks will be secured on truck after extraction for <br />transportation. <br />Tanks and plumbing will be hauled to by Redfern Trucking, 3736 S. Highway 99, <br />Stockton, Ca 95205, (209) 948-0080 to Triangle Inc. of Sacramento, 3525 52nd <br />Avenue, Sacramento, Ca 95823, (916) 421-1990 to be destroyed. All of the <br />above procedures will be done according to code. Western Meter Service, Inc. <br />will mail a copy of the tank manifest to San Joaquin E.P.A. <br />Western Meter Service, Inc. will approach all tank excavation with possible <br />cave-ins.
