The <br /> missing alarm include: L7 Diesel STP sump fuel alarm on 3/25/2017. During the monthly inspection,the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 211 CCR 2715(f)(2) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training log for all current employees is current but is lacking the hire dates. The <br /> list shall include the dates of training for all facility employees and the hiring dates for all facility employees hired on <br /> or after July 1, 2005. Ensure that employees have been trained by the designated operator, maintain the list on site, <br /> and submit a copy of the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 301 HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> Fuel filters were observed outside of all under dispenser containments at facility.The underground storage tank <br /> system shall be operated to prevent unauthorized releases.The underground storage tank system shall be operated <br /> to prevent unauthorized releases. Immediately contact a properly licensed, trained, and certified contractor to <br /> replace or upgrade the component under permit and inspection with the EHD. <br /> This is a Class II violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the under dispenser containment(UDC)sumps in#5/6 and#17/18. If water could enter into <br /> the secondary containment by precipitation or infiltration, it must be removed and disposed of properly.The service <br /> technician removed about 5 cups of liquid from#17/18 UDC but about 1/2 cup remained and#5/6 UDC was not <br /> removed. Ensure that all sumps and annular spaces are maintained free of liquid. Immediately remove this liquid, <br /> make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. <br /> Ensure that the UDC's#5/6 and#17/18 is maintained free of liquid. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by February 2, 2018. <br /> Documents reviewed: Monitoring system certification testing report 2016 and 2017, Secondary Containment <br /> Testing report 2015, Employee training log, designated operator monthly reports, alarm history and alarm logs, <br /> FA0007059 PR0505867 SCO01 01/03/2018 <br /> EHD 23-01 Rev.10/31/2017 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
