UST Facility Operating Permit Application and UST Tank Information for 87 Master, 87 Slave, 91 and diesel are not current in CERS. The UST Facility Operating Permit Application and all four UST Tank Information forms do not have a certification date; the Riser Pipe Primary Containment should be changed from none to'Steel' and the Riser Pipe Secondary Containment should be changed to'None' on all four tank forms; and Tank ID numbers for all four tanks need to be added. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. The monitoring plan is not current and/or not approved by the EHD. The monitoring plan does not have a signature date. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 205 CCR 2712(i) Operating permit not on site. A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. This is a minor violation. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to document all the alarms from the attached alarm history on the January 2015, April 2015, June 2015 and December 2015 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: Tank 1, 2, 3, and 4 leak alarm on January 22, 2015, the sensor L7 diesel STP sump fuel alarm on April 7, 2015, tank 3 (product 91) sudden loss alarm on June 1, 2015, and the cold start alarms on December 17, 2015. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Also the secondary containment date should be October 14, 2015, not October 23, 2012, on the designated operator's report for October and November 2015. This is a minor violation.
