PUBLIC HIALTH SERVIAS
SAN JOAQUIN COUNTY
ENVIRONMENTAL HEALTH DIVISION
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue, Third Floor • Stockton, CA 95202
209/468-3420
tZFakM19
September 22, 1997
USA Petroleum
David Oldfield
1261 E. 9th St.
Pomona CA 91766
RE: Underground storage tank installation application for 2500 W. Lodi Ave.
David,
The installation application for the above facility has been approved with the following
conditions:
1. All appointments shall be made 48 hours in advance.
2. A tank and product line tightness test is required after the installation is
complete and the station is operational.
3. State Water Resources Control Board form "C shall be submitted after the
installation is completed.
4. Refer to the attached conditions of approval for the Geoflex II piping.
5. The buoyancy calculations show that the tanks will not be subject to floatation
if the elevation of the water table is 98 inches. This department will not take
responsibility for the potential float out of the tanks if the tank excavation
should become flooded. Proper engineering practices should be applied so
the tanks do not have the potential to float out of the tank excavation.
If you have any questions regarding this matter please contact me at (209) 468-0333.
L'
Eric Trevena, R.E.H.S.
A Division of San Joaquin County Health Care Services
