Attachment D <br /> NOTICE OF EMPLOYEE JOB HAZARDS <br /> The personal safety and health of each employee of Tank Excavators is of <br /> primary importance . In order to prevent occupational injuries and illnesses , <br /> each employee must be aware of those hazards which he/she may be exposed <br /> to during the performance of his/her job. The Company has listed below those <br /> hazards to which you may be exposed. Please help us by adding to this list if <br /> you are aware of other hazards not listed below. <br /> Job Category: Heavy Equipment Operator/Manual Labor <br /> Heavy Equipment Operation: The safety hazards associated with the <br /> operation of heavy equipment can be effectively eliminated if you maintain <br /> constant awareness of the situation. <br /> Slip-Trip-Fall Hazards: While it is difficult to prevent slip-trip-fall hazards, risk of <br /> injury will be minimized by keeping the work area free of obstructions, wearing <br /> the proper foorwear, and by giving consideration to dangerous work areas. <br /> Lifting Hazards: Field operations often require that heavy physical labor tasks <br /> be performed. Do not attempt to lift large or heavy objects without assistance, <br /> and always use the proper lifting technique (bend knees, not back). <br /> Tool and Equipment Hazards: Safety hazards present during the use of tools <br /> and equipment are generally associated with ifiproper tool handling and <br /> inadequate maintenance. Always maintain your equipment, and if you don't <br /> understand equipment handling techniques, ask your supervisor. <br /> Open Excavations: Open excavations may be present during tank removal <br /> operations. Excavations will be maintained in accordance with OSHA <br /> regulations for confined spaces, but always be aware of the potential hazard <br /> associated with open excavations. <br /> Noise Levels: Hearing protection is provided and should be worn at all times <br /> when noise levels warrant. Equipment operators should routinely wear hearing <br /> protection. <br /> Weather: Excessive heat or cold can cause physical discomfort and personal <br /> injury. Be aware that protective clothing decreases the body's ability to cool <br /> itself. If you experience excessive sweating, dizziness or nausea, stop <br /> immediately. Cool yourself, drink fluids and inform your supervisor. Be aware of <br /> the affect of heat on co-workers. <br /> Chemical Exposure: The primary chemical exposure hazard is to gasoline <br /> fumes. Read the project health and safety plan to know if any chemical <br /> exposure exists at the job site, and take necessary respiratory precautions. <br />
