Financial responsibility documents have not been submitted to the San Joaquin County <br />Environmental Health Dept (EHD). Current financial responsibility documents are required to be <br />submitted annually. Complete and submit a copy of the financial responsibility by April 28, 2013. <br />109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing <br />were last performed on 1-27-12 and was due by January 2013. Partial testing was done Sept 2012. <br />Testing has not been completed and is two months past due. These tests are required once every 12 <br />months, Immediately schedule these tests and provide 48 hours notification to the EHD. Please be <br />aware that a follow up inspection will be charged at our current hourly rate. <br />113. New designated operators Steven Abano and Jason Haase were hired on 2-16-12 and 1-7-13 <br />respectively and notification was not provided to the EHD within 30 days of the change. Any designated <br />operator changes shall be reported to the EHD with 30 days. Submit notification to the EHD identifying <br />all the designated operators for this facility by April 28, 2013. <br />201. Alarm log records were not found on site for the following alarms: <br />a. L2 - 89 STP 4-13-12 fuel alarm <br />b. L3 - 87 STP 4-21-12 fuel alarm <br />c. L1 - 91 STP 4-25-12 fuel alarm <br />These records shall be kept on site for at least 3 years. Document each alarm and explain how each <br />alarm was corrected. Immediately locate and maintain the alarm Nog record for these alarms on site and <br />submit copies to the EHD by April 28, 2013. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL HE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY 'RATE. <br />EHD 23-0 F Rev 04119112 &--'P'-'4(3 3 4�T INSPECTION REPORT <br />
