CLASS it. or MINOR -Notice to Comply] <br />204. The designated operator failed to check that the following alarms were responded to appropriately: <br />a. L2 - 89 STP 4-13-12 fuel alarm <br />b. L3 - 87 STP 4-21-12 fuel alarm <br />c. L1 - 91 STP 4-25-12 fuel alarm <br />During the monthly inspection, the designated operator shall review the alarm history for the previous <br />month, check that each alarm was documented and responded to appropriately, and attach a copy of <br />the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. <br />204. The designated operator failed to check and note the correct test dates for the monitor system <br />certification and the secondary containment test on the past 12 months of designated operator monthly <br />inspection reports. The correct test dates are annual monitoring system certification = 1-27-12 and <br />triennial secondary containment test = 10-24-11. During the monthly inspection, the designated <br />operator shall check that all required testing and maintenance for the UST system have been completed <br />and document the dates they were done. Ensure that designated operators performing monthly <br />inspections at this facility are including all of the required information on the reports. <br />206. The designated operator employee training record was not found on site for training within the past <br />12 months. The designated operator shall train facility employees for which he or she is responsible in <br />the proper operation and maintenance of the UST system once every 12 months. The training shalt/ <br />include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by April 28, 2013. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT TIME AT EHD'S CURRENT HOURLY RATE. <br />P. -i -H RV. i I Title: <br />Garrett Backus (209) 468-2986 <br />EHD 23-02-003 Rev 04/19112 CONTINUATION FORM <br />
