Diesel fuel was observed today in the sump below dispenser 1. The piping did not appear to be <br />leaking. Any release from the primary containment that does not escape the secondary containment <br />and was cleaned up within eight hours of being detected shall be recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br />primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's <br />monitoring records. Submit a copy of the recordable release statement to the Environmental Health <br />Department (EHD) by February 27, 2012. <br />302 & 304. The 89191/diesel split tank annular sensor failed to activate an audible and visual alarm when <br />tested and, therefore, the tank is not monitored. The interstitial space shall be monitored with a <br />monitoring system connected to an audible and visual alarm system. The sensor was replaced and <br />retested good during the inspection. VR 794380-420, SfN 903826 sensor was added to the SR0064044 <br />permit. <br />Notes <br />1. Complete the following by February 27, 2012 and send to the San Joaquin County Environmental <br />Health Department, <br />a. The Return to Compliance Certification form. <br />b. A statement explaining how each violation was corrected. <br />c. Supporting documentation. <br />2. The 87 turbine sump sensor Veeder Root 794380-208, SIN 871440 tested good. Replaced under <br />permit SR0064044, <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL 8E BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECT N AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />f <br />.EH023-O liev 09101/11 Page 3 FIST INSPECTION REPORT <br />
