One liter of fuel was removed from the 87 turbine sump by the technician on May 23, 2013, and <br /> was not recorded. Any release from the primary containment that does not escape the secondary <br /> containment and was cleaned up within eight hours of being detected shall be recorded with the <br /> following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br /> primary containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's <br /> monitoring records. Submit a copy of the recordable release statement to the EHD by June 23, 2013. <br /> 206. Nirmaljit Badial stated that he was hired in 2007 and Jyoti Kamboj states that she was hired in <br /> 2011, but the documentation for the employees trained by the designated operator does not indicate <br /> these employees' hire dates. Vanessa Coss stated that she was hired at least a month ago and Abdur <br /> Khan states that he was hired at least a couple months ago. Vanessa and Abdur where left to work <br /> alone in the store for a couple hours. The facility lacks documentation that Vanessa and Abdur where <br /> trained by the facility designated operator. The designated operator shall train facility employees for <br /> which he or she is responsible in the proper operation and maintenance of the UST system once every <br /> 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> A list of facility employees who have been trained by the designated UST operators(s) shall be <br /> maintained on-site or off-site at a readily available location, if approved by the local agency. The list shall <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector.
