Hwy 99 Stockton, CA 95215 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> The list shall be provided to the local agency upon request. The list shall include the dates of training for <br /> all facility employees, and the hiring dates for all facility employees hired on or after July 1, 2005. Ensure <br /> that employees have been trained by the designated operator, maintain the correctly completed list on <br /> site, and submit a copy of the training records to the EHD by June 23, 2013. <br /> 310. Liquid was observed in the 87, 89, and 91 turbine sumps. If liquid could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Take the <br /> necessary precautions to ensure that the secondary containment of the UST system is maintained free <br /> of liquid. During the inspection the technician removed the liquid from this sump. Continue to ensure that <br /> any liquid from the secondary containment sumps are managed properly per California Title 22 <br /> Hazardous Waste Regulations. <br /> 302 & 308. The test hose in the UDCs were clamped off, and the test boots were left on the coflex <br /> product piping in the UDCs. Any potential release from the primary containment into the secondary <br /> containment in the fuel piping does not flow into the UDC sumps. All secondary containment systems <br /> shall be sloped so that all releases will flow to a collection sump located at the low point of the <br /> underground piping. Test boots should be loose or pulled back, and test hoses left unclamped so that <br /> releases into the product piping secondary can reach the closest sump and the UST monitoring system <br /> will be able to detect a leak at the earliest opportunity. Immediately contact a properly licensed, trained, <br /> and certified contractor to ensure that any releases in the product piping flows into all the facility UDCs. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by June 23, 2013. <br /> ALL EHD STAFF TPME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received Bye Title: <br /> Aris Cacapit (209) 468-3453 <br /> EHD 23-02-003 Rev 0419/32 CONTINUATION FORM <br />
