0 0 <br /> BANKS&CO. <br /> C. No employee shall ever place himself or another within an <br /> excavation which is doubted to be safe . <br /> D. Any trench over 4 ' in depth must be shored by at least <br /> the minimum requirements or sloped back to at least a 3/4 <br /> 1 angle, or as conditions require . <br /> E . An authorized qualified supervisor shall direct the <br /> shoring operation and modify as needed to insure maximum <br /> worker safety. <br /> F. At the beginning of each shift , the trench and shores <br /> shall be inspected for any unsafe conditions prior to <br /> anyone entering the excavation. <br /> G. A ladder will be used in all excavations and shall be <br /> within 25 feet (horizontal distance) of any workmen in <br /> the trench. The ladder shall be firmly positioned from <br /> the bottom on one side of the trench to the top of the <br /> opposite side and extend at least 2 ' above the ground <br /> level surface . At no time shall the ladder be placed <br /> upon the shores or upon any pipe being installed within <br /> the trench. <br /> H. Existing utilities shall be located and marked. Utility <br /> owners shall be notified 48 hours in advance prior to <br /> trenching within the area of their lines . <br /> I . Always be alert to the presence of high-voltage lines and <br /> never operate any piece of equipment (i .e. loader bucket, <br /> backhoe boom) within 10 feet of such lines . <br /> J. No unnecessary machinery or heavy loads will be permitted <br /> along the edges of a trench. If it becomes necessary to <br /> dump or lower material into the trench, the workmen below <br /> shall vacate to a safe area until such procedure has been <br /> complete . <br /> K. If your task is complete in the trench, don ' t loiter! <br /> Vacate to the top side . <br /> L. Gases may exist at any time within the ground. It may be <br /> from pockets of decaying matter or a leaky gas main in <br /> the vicinity. A test will be made to see if gases are <br /> present but please don' t smoke when you are in the <br /> trench. <br /> 20 SITE SAFETY PLAN <br />
