The <br />missing alarms include: <br />Alarm missing from 3124/17 DO monthly inspection report: <br />41 Premium PLLD Shutdown Alarms on 3/23117 <br />Alarms missing from 4128/17 DO monthly inspection report: <br />Q1 Premium PLLD Shutdown Alarms on 3126117 <br />Q3 Regular PLLD Shutdown Alarms on 418117 <br />Alarms missing from 6130/17 DO monthly inspection report: <br />T1 Premium Center Leak Alarm on 7/24117 <br />T3 Regular South Leak Alarm on 7124/17 <br />Q1 Premium PLLD Shutdown Alarm on 7/31/17 <br />Alarms missing from 8/4/17 DQ monthly inspection report: <br />L5 Diesel Annular High Liquid Alarm on 9/8117 and 9/11/17 <br />During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reports. <br />This is a minor violation. <br />319 CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to prcvide a means to keep it empty. <br />The diesel -product spill container failed the one-hour hydrostatic test. All LISTS shall be equipped with a spill <br />container that has a minimum capacity of five gallons and a drain valve that allows drainage of the collected spill into <br />the primary container or provide a means to keep the container empty. The service technician discovered that the <br />adapter was loose, tightened the adapter, and retested the spill container. The spill container passed the one-hour <br />hydrostatic test. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 312/18. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />EHD 23-01 Rev. 09122116 <br />Page 5 of 6 <br />FA0003994 PR0231785 SC001 0113112018 <br />Underground Storage Tank Program CIR <br />
