The site plan does not show the approximate location of <br />the underground storage tank piping. A site plan must be submitted identifying the locations where monitoring will <br />be performed. Immediately log into the California Environmental Reporting System (CERS) at <br />http:llcers.calepa,ca.gov/ and upload a copy of the UST Monitoring Site Plan. <br />This was corrected on site. <br />This is a repeat violation, Class Il. <br />110 <br />CCR 2637(e) Failed to submit secondary containment test report within 30 days. <br />Secondary containment testing was performed on 11129/2017 and a test report was submitted on 112312018, over <br />one month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary <br />precautions to ensure testing and submittal of test reports are performed in a timely manner. <br />This is a minor violation. <br />201 <br />CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br />Maintenance and monitoring records for the last three years were not found on site. The facility alarm log did not <br />document nor address the T1 Premium Center Leak Alarm that occurred on 7124117. A maintenance record <br />addressing this alarm was could' not located at the time of inspection. These records shall be maintained on site for <br />at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring <br />equipment Calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis <br />performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water <br />elevations, or other test results; (6) results of inventory readings and reconciliations.
