Therefore, the EHD
directs that you proceed with the well destructions scheduled for August and September 2014 if
in compliance with the conditions presented in the EHD letter dated 16 April 2014.
Please submit an EHD Well and Boring Permit Application form along with a Master File Record
form, encroachment permit issued by the City of Stockton, access agreement from the adjacent
property owner where an off-site well is located and appropriate fees for each parcel. The
permit fee for the destruction of each well is $125, along with a $375 Inspection Activity fee per
parcel. A review of the site records indicate that there are twenty monitoring wells, two vapor
extraction wells, seven groundwater extraction wells and six ozone sparge wells associated with
this site. A fee of $5,500.00 (35 x $125 + $375 x 3) will be required for the well destruction
permits.
Please contact Vicki McCartney at (209) 468-9852 or vmccartney@sjcehd.com if you have any
questions.
lSinnccereellyy,,
Victoria McCartney, REHS Nuel C.
