The owner or operator does not maintain aisle space to allow <br /> the unobstructed movement of personnel or fire protection <br /> equipment to any area of the facility operation in an <br /> emergency. Section 66265. 35, Article 3 , Chapter 15, Title 22 , <br /> CCR. <br /> 13 . Personnel training records lacked the following; <br /> (1) the job title for each position at the facility related to <br /> hazardous waste management, and the name of each employee <br /> filling that position; <br /> (2) a written job description for each position listed under <br /> subsection (d) (1) of this section. <br /> (3) a written description of the type and amount of both <br /> introductory and continuing training that will be given to <br /> each person filling a position listed under subsection (d) (1) <br /> of this section. <br /> (4) records that document the training or job experience <br /> required under subsection (a) , (b) , and (c) of this section <br /> has been given to, and completed by, facility personnel. <br /> Section 66265. 16, Article 2 , Chapter 15, Title 22 , CCR. <br /> 14 . The contingency plan lacked the following; <br /> (1) The plan does not describe arrangements agreed to by local <br /> police departments, fire departments, hospitals, contractors, <br /> and State and Local emergency response teams to coordinate <br /> emergency services, pursuant to section 66265. 37. Section <br /> 66265. 52 (c) , Article 4, Chapter 15, Title 22 , CCR. <br /> (2) The plan does not list names, addresses, and phone numbers <br /> (office and home) of all persons qualified to act as emergency <br /> coordinator. Section 66265. 52 (d) , Article 4 , Chapter 15, <br /> Title 22 , CCR. <br /> (3) The plan does not list all the emergency equipment at the <br /> facility [such as fire extinguishing systems, spill control <br /> equipment, communication and alarm systems (internal and <br /> external) , and decontamination equipment] , . This list does not <br /> include the location and a physical description of each item <br /> on the list, and a brief outline of its capabilities. Section <br /> 66265. 52 (e) , Article 4, Chapter 15, Title 22 , CCR. <br /> 15 . Hazardous waste treatment and storage appeared less than 50 ft <br /> from property lines. Section 66265. 175, Title 22 , CCR. Health <br /> and Safety Code Section 25189. 5 <br /> 16. Open tank of untreated hazardous waste was found overfilled, <br /> which is required to be less than 60 centimeters (two feet) <br /> from the top of the container.
