Chapter 15, Title 22, CCR. <br /> 13. Personnel training records lacked the following; <br /> Section 66265.16, Article 2, Chapteer 15, Title 22, CCR. <br /> (d) (1) the job title for each position at the facility related to hazardous waste <br /> management, and the name of each employee filling that position; <br /> (d) (2) a written job description for each position listed under subsection (d) (1) <br /> of this section. <br /> (d) (3) a written description of the type and amount of both introductory and <br /> continuing training that will be given to each person filling a position listed under <br /> subsection (d) (1) of this section. <br /> (d) (4) records that document that the training or job experience required under <br /> subsection (a), (b), and (c) of this section has been given to, and completed by, <br /> facility personnel. <br /> 14. The contingency plan lacked the following; <br /> (1) The plan does not describe the actions facility personnel shall take to comply <br /> with sections 66265.51 and 66265.56 in response to fire, explosions, or any <br /> unplaanned sudden or non-sudden release of hazardous waste or hazardous <br /> waste constituents to air, soil, or surface waaaater at the faaacility. Section <br /> 66265.52 (a), Article 4, Chapter 15, Title 22, CCR. <br /> (2) The plan does not describe arrangements agreed to by local police <br /> departments, fire departments, hospitals, contractors, and State and Local <br /> emergency response teams to coordinate emergency services, pursuant to section <br /> 66265.37. Section 66265.52 (c), Article 4, Chapter 15, Title 22, CCR. <br /> (3) The plan does not list names, addresses, and phone numbers (office and <br /> home) of all persons qualified to act as emergency coordinator. Section 66265.52 <br /> (d), Article 4, Chapter 15, Title 22, CCR. <br /> (4) The plan does not list all the emergency equipment at the facility (such as fire <br /> extinguishing systems, spill control equipment, communication and alarm systems <br /> (internal and external), and decontamination equipment), where this equipment is <br /> required. This list does not include the location and a physical description of each <br /> item on the list, and a brief outline of its capabilities.
