Specific violations <br /> and corrective actions are listed below. Failure to correct the <br /> identified violations within the schedule provided will result in <br /> PHS-EHD citing you for continuing/additional violations. <br /> I. REPORT OF VIOLATIONS: <br /> 1. Title 22, CCR, Section 66268.7. At the time of <br /> inspection, manifest 188332411, which requires a Land <br /> Disposal Restriction Notification Form, did not have a <br /> Land Disposal Restriction Form attached. <br /> 2 . Title 22, CCR, Section 66265. 196. At the time of <br /> inspection, the protective coating of the bermed <br /> secondary containment area for the hazardous waste <br /> treatment unit was ruptured and corroded. <br /> 3 . Health and Safety Code, Section 25244 .21. At the time <br /> of inspection, a copy of the current Source Reduction <br /> Evaluation Review and Plan, Source Reduction Evaluation <br /> Review and Plan Summary, Hazardous Waste Management <br /> Performance Report, and Hazardous Waste Management <br /> Performance Report Summary on hazardous waste <br /> minimization (pursuant to Sections 25244. 19. and <br /> 25244. 20. ) were not available for review. <br /> A Division of San Jmquin County Hn1th Gare] mct <br />
