Violation 15: Within 20 days of receipt of this report, a <br /> training program specifically addressing hazardous waste <br /> handling shall be implemented. <br /> Violation 06: Within 30 days of receipt of this report, a <br /> Contingency Plan shall be submitted to DHS and PHS-EHD. A copy of <br /> the plan must be accessible at the facility at all times. <br /> Violation 17: Within 30 days of receipt of this report, an <br /> Exception Report shall be submitted to DHS and PHS-EHD. <br /> When all violations have been corrected, please send written <br /> certification to this office at the letterhead address.
