Section 66265. 54, Division 4.5, Title 22, CCR: <br /> Contingency plan was not amended to reflect recent <br /> personnel changes. <br /> II SCHEDULE OF CORRECTIONS: <br /> Violation #1: Within seven days of receipt of this report, <br /> remove all hazardous waste that has exceeded <br /> the allowable accumulation time under manifest <br /> by a California hazardous waste hauler to an <br /> approved site. <br /> Violations #2 Effectively immediately, all hazardous waste <br /> #3 , #4, #5, containers shall be labeled with the following <br /> #6, #7, #8 & information: <br /> #9 <br /> a) The start date of accumulation <br /> b) The words "HAZARDOUS WASTE" <br /> C) Composition and physical state of the <br /> waste. <br /> d) Statement(s) which will call attention to <br /> the particular hazardous properties of <br /> the waste. <br /> e) Name and address of person producing the <br /> waste. <br /> Violation #10 Within seven days of receipt of this report, <br /> amend the necessary portions of the <br /> contingency plan according to referenced code <br /> section. <br /> When all violations have been corrected, please send written <br /> certification to this office at the letterhead address. This <br /> certification shall contain the sample certification statement <br /> indicated on Attachment I. <br /> PHS-EHD may conduct a follow-up inspection at the facility to <br /> verify compliance. <br /> The issuance of this report of violations and schedule of <br /> correction does not preclude PHS-EHD from taking any <br /> administrative, civil, or criminal action as a result of the <br /> violations noted herein. <br /> If you have any questions or should require additional <br /> information, contact Eric Trevena, REHS at (209) 468-0333 . <br />
